Property Damage Claim Form

Please report the incident to the PWC Claims Department by submitting a detailed description of what occurred and a completed claims form, which is available below. Emailing your claim and supporting documentation is preferred and can be sent to claims@faypwc.com.

To assist with processing your claim, you should provide an inventory of all damages and have it available for assessment during the investigation. Please photograph or video any items that were disposed of (e.g., food spoilage or water/sewage damage).

All supporting documentation, including receipts and evidence of damages, should be submitted via email. Any questions regarding your claim may also be directed to claims@faypwc.com.
